Starting Pay: $40.03 to $50.04 DOE

 

Full-time, benefited position work 80 hours in an exempt status. Typical days of work are Monday - Friday with hours from 8 am - 4:30 pm. A background check is required.

 

The Home Care Director, a registered nurse, provides exemplary leadership in nursing practice and oversees ancillary disciplines within Home Health and Hospice. This role is pivotal in guiding the agency towards regulatory compliance, spearheading quality and performance improvement initiatives, and ensuring robust infection control practices. The Home Care Director also manages overall agency operations and serves as a clinical resource for all staff members within the Home Care agency, fostering a supportive and effective work environment.

Responsibilities

Quality 

  • Maintains the standards of nursing care and implements the policies and procedures of the Home Health and Hospice agency.  Reviews and revises policies and procedures based on evidence-based practice and in alignment with regulatory standards.
  • Maintains high level of knowledge regarding current CMS Conditions of Participation and Joint Commission standards, and ensures that the agency is compliant with these requirements.
  • Responsible to develop and lead ongoing programs to monitor, evaluate, and improve the quality and appropriateness of patient care as required by regulatory guidelines, and/or as identified by performance data. Collaborates with Home Health Coordinator and Hospice Coordinator to develop and maintain the Home Care QAPI program. 
  • Assures the appropriate use of the nursing process in the delivery of care to all ages of patients including geriatric, adult, adolescent and pediatrics.
  • Acts as liaison between patient, family and physician and other members of the health team.
  • Provides continuity of care and discharge planning which is integrated with other health disciplines.
  • Participates in the development of organizational and departmental goals and implements processes to meet those goals.

Service

  • Ensures that care is delivered to each patient in accordance with quality standards and is documented in clinical records.
  • Acts as a liaison with long-term care facilities, physicians, and families in provision of Hospice care in collaboration with Hospice Coordinator.
  • Represents JRMC Home Health and Hospice as required within community for educational or public relation needs.
  • Reports and follows-up on potential risk management, corporate compliance, HIPAA, and quality and safety issues on a timely basis and reports findings to VP of Clinical Services.

 People

  • Supervision of Home Health and Hospice personnel to include nursing, social work, home health aide, physical and occupational therapists, and Hospice Chaplain, including ensuring that personnel meet all requirements to maintain compliance.
  • Responsible for the recruiting, interviewing, and selection of staff. Utilizes peer interviewing process. 
  • Responsible for development and adjustment of staffing structure based on census and growth of the agency.
  • Responsible for the orientation planning, scheduling, and overall onboarding experience of all new Home Care staff. Participates in staff development programs. 
  • Guides staff in the process of personal/professional growth through coaching, counseling, and mentoring.
  • Evaluates the competence of staff using established performance standards; communicates corrective action to VP of Clinical Services and ensures follow up is completed.
  • Responsible to make or provide input into making the employee schedule for department. Implements staffing policies and procedures to ensure adequate coverage of patient care.
  • Participates in Quarterly Leadership Development training.
  • Serves on committees and attends meetings as assigned or requested by VP of Clinical Services.

Finance

  • Serves as a resource for collection of OASIS data and maintains competency of OASIS skill throughout all changes to the OASIS dataset.  Responsible for maintaining OASIS competency program within the department to ensure the highest quality of data is being obtained.
  • Responsible for preparing in preparing the annual budget for Home Health, Hospice, and Private Duty.
  • Responsible for preparing annual report for Home Health and Hospice.

Growth

  • Assists VP of Clinical Services in planning programs that are integrated with patient care delivered within the agency
  • Participates in state associations for Home Health and Hospice.
  • Performs the duties and responsibilities of staff RN.
  • Performs other duties as assigned or requested. 

Qualifications

Work requires a professional level of knowledge acquired by completing an accredited RN nursing education program. Candidates must hold RN licensure from the ND board of Nursing for the current year. Preference for a Baccalaureate Degree in Nursing.

  • Minimum of three years of general nursing experience in hospital (health care field).
  • Preference for previous supervisory or management experience.
  • Knowledge of Joint Commission and CMS Standards.
  • Knowledge of administrative nursing practice.
  • Time to reach competency on the job: 3 months to 6 months following orientation to the work area.
